Self-Pay Rates:
*I can provide a superbill upon request for potential reimbursement with your insurance provider. Please call your insurance provider to confirm out-of-network benefits.
Initial OT or OT/Lactation Office Evaluation (90 minutes): $350
You will be provided with a thorough evaluation report and care plan to share with your family and medical team
Follow Up Office Appointments (60 minutes): $175
Detailed care plan provided after every appointment

**Home visits are available within a 10 mile radius of the Laguna Hills office for an additional fee
Insurance Coverage:
Please note that I am in-network with several insurance plans under lactation services. I am out-of-network for occupational therapy services. If lactation/feeding are not addressed during the appointment (i.e. occupational therapy-only visits), we are unable to bill insurance directly. However, a superbill can be provided upon request.
I am directly in-network with Aetna for lactation support. I also partner with trusted third-party billing organizations to connect insurance benefits with lactation services:
Wildflower – provides lactation coverage for Cigna members
MilkWise – provides lactation coverage for United Healthcare PPO plans
How Coverage Works:
If your insurance plan is in-network with Aetna, MilkWise (United PPO), or Wildflower (Cigna):
The initial 90-minute office evaluation includes both a lactation and an occupational therapy assessment/treatment. Insurance will be billed only for the lactation portion. The occupational therapy portion of the visit is not covered by insurance and is subject to a separate $150 fee. A superbill can be provided for this additional fee.
For 30-60 minute follow-up appts, the appt will be billed to insurance under lactation only - as long as we are addressing infant feeding/lactation during the appointment.

Telehealth: Covered only for Wildflower (Cigna) clients at this time
Home Visits: Limited availability within a 10-mile radius of the Laguna Hills office for an additional fee
Home Visit Travel Fee:
0-10 mile radius from Laguna Hills office: $100
**I am not currently seeing clients for home visits outside of a 10 mile radius. However, I am happy to connect you with a provider in your area!
